2019 ACTC Post Grant Follow up (Due January 1st, 2020 at 11:59pm)

As an organization that received a grant in January 2019, you are required to provide information regarding the use of your funds as well as the success of the project or program that was funded.

Completion of this form is required to be complete and submitted by January 1st, 2020  at 11:59pm.

Failure to complete a Post Grant Follow up Report on the use of the grant funds will render you ineligible for future funding, effective immediately for the 2019 season.
